It turns out this is not as NFC as we had hoped, because operator== will consider two std::error_codes to be distinct even though they both hold "success" values if they have different categories.

raw_ostream: add operator<< overload for std::error_code
Summary: The main motivation for this is unit tests, which contain a large macro for pretty-printing std::error_code, and this macro is duplicated in every file that needs to do this. However, the functionality may be useful elsewhere too.
In this patch I have reimplemented the existing ASSERT_NO_ERROR macros to reuse the new functionality, but I have kept the macro (as a one-liner) as it is slightly more readable than ASSERT_EQ(..., std::error_code()).

Update the file headers across all of the LLVM projects in the monorepo to reflect the new license.
We understand that people may be surprised that we're moving the header entirely to discuss the new license. We checked this carefully with the Foundation's lawyer and we believe this is the correct approach.
Essentially, all code in the project is now made available by the LLVM project under our new license, so you will see that the license headers include that license only. Some of our contributors have contributed code under our old license, and accordingly, we have retained a copy of our old license notice in the top-level files in each project and repository.

This breaks the OpenFlags enumeration into two separate enumerations: OpenFlags and CreationDisposition. The first controls the behavior of the API depending on whether or not the target file already exists, and is not a flags-based enum. The second controls more flags-like values.
This yields a more easy to understand API, while also allowing flags to be passed to the openForRead api, where most of the values didn't make sense before. This also makes the apis more testable as it becomes easy to enumerate all the configurations which make sense, so I've added many new tests to exercise all the different values.

Re-sort #include lines for unittests. This uses a slightly modified clang-format (https://reviews.llvm.org/D33932) to keep primary headers at the top and handle new utility headers like 'gmock' consistently with other utility headers.
No other change was made. I did no manual edits, all of this is clang-format.
This should allow other changes to have more clear and focused diffs, and is especially motivated by moving some headers into more focused libraries.

Fix a real temp file leak in FileOutputBuffer
If we failed to commit the buffer but did not die to a signal, the temp file would remain on disk on Windows. Having an open file mapping and file handle prevents the file from being deleted. I am choosing not to add an assertion of success on the temp file removal, since virus scanners and other environmental things can often cause removal to fail in real world tools.
Also fix more temp file leaks in unit tests.

Don't allow pwrite to resize a stream.
The current implementations could exhibit some behavior differences:
raw_fd_ostream: Whatever the underlying fd does with seek+write. In a normal file, the write position would be back to the old offset.
raw_svector_ostream: The write position is always the end of the stream, so after pwrite the write position would be the new end. This matches what OS_X (all BSD?) do with a pwrite in a O_APPEND fd.
Given that we don't need that feature and don't use O_APPEND a lot in LLVM, just disallow it.
I am open to suggestions on renaming pwrite to something else, but this fixes the issue for now.
