[Transforms] Use StringRef::operator== instead of StringRef::equals (NFC) (#91072)

I'm planning to remove StringRef::equals in favor of
StringRef::operator==.

- StringRef::operator==/!= outnumber StringRef::equals by a factor of
31 under llvm/ in terms of their usage.

- The elimination of StringRef::equals brings StringRef closer to
std::string_view, which has operator== but not equals.

- S == "foo" is more readable than S.equals("foo"), especially for
!Long.Expression.equals("str") vs Long.Expression != "str".

[Support] Make report_fatal_error respect its GenCrashDiag argument so it doesn't generate a backtrace

There are a few places where we use report_fatal_error when the input is broken.
Currently, this function always crashes LLVM with an abort signal, which
then triggers the backtrace printing code.
I think this is excessive, as wrong input shouldn't give a link to
LLVM's github issue URL and tell users to file a bug report.
We shouldn't print a stack trace either.

This patch changes report_fatal_error so it uses exit() rather than
abort() when its argument GenCrashDiag=false.

Make llvm::StringRef to std::string conversions explicit.

This is how it should've been and brings it more in line with
std::string_view. There should be no functional change here.

This is mostly mechanical from a custom clang-tidy check, with a lot of
manual fixups. It uncovers a lot of minor inefficiencies.

This doesn't actually modify StringRef yet, I'll do that in a follow-up.

Sink all InitializePasses.h includes

This file lists every pass in LLVM, and is included by Pass.h, which is
very popular. Every time we add, remove, or rename a pass in LLVM, it
caused lots of recompilation.

I found this fact by looking at this table, which is sorted by the
number of times a file was changed over the last 100,000 git commits
multiplied by the number of object files that depend on it in the
current checkout:
recompiles touches affected_files header
342380 95 3604 llvm/include/llvm/ADT/STLExtras.h
314730 234 1345 llvm/include/llvm/InitializePasses.h

137647 59 2333 llvm/include/llvm/Support/Error.h
131619 73 1803 llvm/include/llvm/Support/FileSystem.h

Before this change, touching InitializePasses.h would cause 1345 files
to recompile. After this change, touching it only causes 550 compiles in
an incremental rebuild.

[CodeExtractor] Factor out and reuse shrinkwrap analysis

Factor out CodeExtractor's analysis of allocas (for shrinkwrapping
purposes), and allow the analysis to be reused.

This resolves a quadratic compile-time bug observed when compiling
AMDGPUDisassembler.cpp.o.

Pre-patch (Release + LTO clang):

```
---User Time--- --System Time-- --User+System-- ---Wall Time--- --- Name ---
176.5278 ( 57.8%) 0.4915 ( 18.5%) 177.0192 ( 57.4%) 177.4112 ( 57.3%) Hot Cold Splitting
```

Post-patch (ReleaseAsserts clang):

```
---User Time--- --System Time-- --User+System-- ---Wall Time--- --- Name ---
1.4051 ( 3.3%) 0.0079 ( 0.3%) 1.4129 ( 3.2%) 1.4129 ( 3.2%) Hot Cold Splitting
```

Testing: check-llvm, and comparing the AMDGPUDisassembler.cpp.o binary
pre- vs. post-patch.

An alternate approach is to hide CodeExtractorAnalysisCache from clients
of CodeExtractor, and to recompute the analysis from scratch inside of
CodeExtractor::extractCodeRegion(). This eliminates some redundant work
in the shrinkwrapping legality check. However, some clients continue to
exhibit O(n^2) compile time behavior as computing the analysis is O(n).

[BlockExtractor] Extend the file format to support the grouping of basic blocks

Prior to this patch, each basic block listed in the extrack-blocks-file
would be extracted to a different function.

This patch adds the support for comma separated list of basic blocks
to form group.

When the region formed by a group is not extractable, e.g., not single
entry, all the blocks of that group are left untouched.

Let us see this new format in action (comments are not part of the
file format):
;; funcName bbName[,bbName...]
foo bb1 ;; Extract bb1 in its own function
foo bb2,bb3 ;; Extract bb2,bb3 in their own function
bar bb1,bb4 ;; Extract bb1,bb4 in their own function
bar bb2 ;; Extract bb2 in its own function

Assuming all regions are extractable, this will create one function and
thus one call per region.

Update the file headers across all of the LLVM projects in the monorepo
to reflect the new license.

We understand that people may be surprised that we're moving the header
entirely to discuss the new license. We checked this carefully with the
Foundation's lawyer and we believe this is the correct approach.

Essentially, all code in the project is now made available by the LLVM
project under our new license, so you will see that the license headers
include that license only. Some of our contributors have contributed
code under our old license, and accordingly, we have retained a copy of
our old license notice in the top-level files in each project and
repository.

In the transition period the DEBUG() macro is still present and aliased
