This patch adds the following pieces to the locale base API:
- __setlocale (for std::setlocale)
- __lconv_t (for std::lconv)
- _LIBCPP_FOO_MASK and _LIBCPP_LC_ALL
This should be sufficient to implement all of the platform-agnostic
localization support in libc++ without relying directly on any public
API names from the C library. This makes it possible to port libc++ to
platforms that don't provide the usual locale APIs.

Our current locale base API is a mix of non-reserved system names that
we incorrectly (re)define and internal functions and macros starting
with __libcpp. This patch introduces a function-based internal interface
to isolate the rest of the code base from that mess, so that we can work
on refactoring how each platform implements the base API in subsequent
patches. This makes it possible to refactor how each platform implements
the base localization API without impacting the rest of the code base.

This patch runs clang-format on all of libcxx/include and libcxx/src, in
accordance with the RFC discussed at [1]. Follow-up patches will format
the benchmarks, the test suite and remaining parts of the code. I'm
splitting this one into its own patch so the diff is a bit easier to
review.

I've left `#include "include/x.h"` and `#include "../y.h"` alone because I'm less sure that they're interchangeable, and they aren't inconsistent with libcxx/include/ because libcxx/include/ never does that kind of thing.
Also, use the `_LIBCPP_PUSH_MACROS/POP_MACROS` dance for `<__undef_macros>`, even though it's technically unnecessary in a standalone .cpp file, just so we have consistently one way to do it.

[libc++] Change workaround for init_priority((100)) outside of system headers
We had previously been using a different workaround for pretending that we were inside a system header, however it had some undesirable effects on dependency parsing for build systems, as explained in [1].
This patch changes the workaround to use `#pragma GCC system_header`, which shouldn't suffer from the same issue. Unfortunately, it is a lot more verbose. The issue is that `#pragma GCC system_header` is ignored when we are inside a source file, so we have to create a header just for the sake of using it. IMO this seems like an artificial restriction without much benefit, but investigating that is a different story. For now, this should at least solve build system problems at the cost of some readability.

Some embedded platforms do not wish to support the C library functionality for handling wchar_t because they have no use for it. It makes sense for libc++ to work properly on those platforms, so this commit adds a carve-out of functionality for wchar_t.
Unfortunately, unlike some other carve-outs (e.g. random device), this patch touches several parts of the library. However, despite the wide impact of this patch, I still think it is important to support this configuration since it makes it much simpler to port libc++ to some embedded platforms.

Based on https://github.com/NuxiNL/cloudlibc, it appears that the CloudABI project has been abandoned. This patch removes a bunch of CloudABI specific logic that had been added to support that platform.
Note that some knobs like LIBCXX_ENABLE_STDIN and LIBCXX_ENABLE_STDOUT coud be useful in their own right, however those are currently broken. If we want to re-add such knobs in the future, we can do it like we've done it for localization & friends so that we can officially support that configuration.

The MSVC CRT uses TLS storage to implement per-thread locales. This storage gets freed during program termination, and if we attempt to do any io operations (like flushing the std streams) after this occurs the program may abort.
This patch is a speculative fix for that issue.
The fix tries forcing the initialization of the locale TLS before initializing the std streams. This should mean that the TLS is freed after we destroy the streams.

Update more file headers across all of the LLVM projects in the monorepo to reflect the new license. These used slightly different spellings that defeated my regular expressions.
We understand that people may be surprised that we're moving the header entirely to discuss the new license. We checked this carefully with the Foundation's lawyer and we believe this is the correct approach.
Essentially, all code in the project is now made available by the LLVM project under our new license, so you will see that the license headers include that license only. Some of our contributors have contributed code under our old license, and accordingly, we have retained a copy of our old license notice in the top-level files in each project and repository.
