Previously, where `name` was applied to a template-specialization TypeLoc, it returned the entire specialization (name, brackets, args) rather than just the name identifier. With this change, exactly the name token is selected.

This is how it should've been and brings it more in line with std::string_view. There should be no functional change here.
This is mostly mechanical from a custom clang-tidy check, with a lot of manual fixups. It uncovers a lot of minor inefficiencies.
This doesn't actually modify StringRef yet, I'll do that in a follow-up.

Summary: The Transformer library has been growing inside of lib/Tooling/Refactoring. However, it's not really related to anything else in that directory. This revision moves all Transformer-related files into their own include & lib directories. A followup revision will (temporarily) add forwarding headers to help any users migrate their code to the new location.

The RangeSelector library defines a combinator language for specifying source ranges based on bound ids for AST nodes. The combinator approach follows the design of the AST matchers. The RangeSelectors defined here will be used in both RewriteRule, for specifying source affected by edit, and in Stencil for specifying source to use constructively in a replacement.
