DEVFS - rollup - Userland, nrelease, and installer
* Remove MAKEDEV and MAKEDEV use in the build system.
* Add a new libc function fdevname() taking a file descriptor and returning the device name.
* Remove the MFS /dev for the CD and adjust the nrelease build system to use devfs.
* Installer adjustments: PFI search path, remove device node maker (it is non-applicable with devfs).
* Add mount_devfs to the build Makefile.
* Change dhclient to use an auto-cloning bpf.
* The VN device now uses labels by default, and no longer supports non-label configurations (though you can of course ignore the label area and just r/w the raw device anyway).

Update installer to 1.1.2.
The most significant change in this update is that there are now two seperate directories on the install medium, /etc and /etc.hdd :
- /etc contains the configuration used for booting the LiveCD only. It is NOT copied to the hard drive.
- /etc.hdd contains the default configuration used for booting from a hard drive. It is NOT used for booting the LiveCD. It IS copied to the hard drive, as /etc .
This avoids the `copy pristine files afterward' rigamarole and, in so doing, simplifies both manual and automatic install routines.
The README file has also been updated to reflect this change.

Split the suggested invocation of fdisk (which is failing for some people) into two seperate invocations (which seems to work for them.)
The real solution would be to find out why fdisk -IB is troublesome, but in the interim, this should save some frustration.

Add a 'dd' prior to the first fdisk to cover situations where people are uprading from very old or extended boot blocks which might not otherwise be completely wiped by fdisk, boot0cfg, and disklabel. Old left over cruft often causes systems to panic with a BTX fault, usually due to bad track/cylinder specifications in the boot sector which cause an overflow (and often a divide by 0) fault in the BIOS.
