`BitcodeModule` is used as element of a vector in `IRSymtabFile`, while in the header there is only a forward declaration. It will work if the header `BitcodeReader.h` is included before including `IRObjectFile.h`. However, it is not always the case, causing compilation error. This patch simply includes the header and remove the forward declaration.

Update the file headers across all of the LLVM projects in the monorepo to reflect the new license.
We understand that people may be surprised that we're moving the header entirely to discuss the new license. We checked this carefully with the Foundation's lawyer and we believe this is the correct approach.
Essentially, all code in the project is now made available by the LLVM project under our new license, so you will see that the license headers include that license only. Some of our contributors have contributed code under our old license, and accordingly, we have retained a copy of our old license notice in the top-level files in each project and repository.

The style guide states that the explicit `inline` should not be used with inline methods. classof is very common inline method with a fair amount on inconsistency:
$ git grep classof ./include | grep inline | wc -l 230 $ git grep classof ./include | grep -v inline | wc -l 257
I chose to target this method rather the larger change since this method is easily cargo-culted (I did it at least once). I considered doing the larger change and removing all occurrences but that would be a much larger change.

We will eventually change IRObjectFile to read from a bitcode symbol table rather than using ModuleSymbolTable, so it would not be able to expose the module.

This change is in preparation for a future change for supporting multiple modules in an IRObjectFile. Although it causes an increase in memory consumption, we can deal with that issue separately by introducing a bitcode symbol table.
