Update the file headers across all of the LLVM projects in the monorepo to reflect the new license.
We understand that people may be surprised that we're moving the header entirely to discuss the new license. We checked this carefully with the Foundation's lawyer and we believe this is the correct approach.
Essentially, all code in the project is now made available by the LLVM project under our new license, so you will see that the license headers include that license only. Some of our contributors have contributed code under our old license, and accordingly, we have retained a copy of our old license notice in the top-level files in each project and repository.

Reorganize the C API headers to improve build times.
Type specific declarations have been moved to Type.h and error handling routines have been moved to ErrorHandling.h. Both are included in Core.h

This gives a lot of love to the docs for the C API. Like Clang's documentation, the C API is now organized into a Doxygen "module" (LLVMC). Each C header file is a child of the main module. Some modules (like Core) have a hierarchy of there own. The produced documentation is thus better organized (before everything was in one monolithic list).
This patch also includes a lot of new documentation for APIs in Core.h. It doesn't document them all, but is better than none. Function docs are missing @param and @return annotation, but the documentation body now commonly provides help details (like the expected llvm::Value sub-type to expect).

Restore other bits of the C API that I tore up. All pre-existing APIs default to using the default global context, while new *InContext() APIs have been added that take a LLVMContextRef parameter.
