[lldb] Make logging machinery type-safe
This patch makes use of c++ type checking and scoped enums to make logging statements shorter and harder to misuse.
Defines like LIBLLDB_LOG_PROCESS are replaces with LLDBLog::Process. Because it now carries type information we do not need to worry about matching a specific enum value with the right getter function -- the compiler will now do that for us.
The main entry point for the logging machinery becomes the GetLog (template) function, which will obtain the correct Log object based on the enum type. It achieves this through another template function (LogChannelFor<T>), which must be specialized for each type, and should return the appropriate channel object.
This patch also removes the ability to log a message if multiple categories are enabled simultaneously as it was unused and confusing.
This patch does not actually remove any of the existing interfaces. The defines and log retrieval functions are left around as wrappers around the new interfaces. They will be removed in follow-up patch.

Summary: A *.cpp file header in LLDB (and in LLDB) should like this: ``` //===-- TestUtilities.cpp -------------------------------------------------===// ``` However in LLDB most of our source files have arbitrary changes to this format and these changes are spreading through LLDB as folks usually just use the existing source files as templates for their new files (most notably the unnecessary editor language indicator `-*- C++ -*-` is spreading and in every review someone is pointing out that this is wrong, resulting in people pointing out that this is done in the same way in other files).
This patch removes most of these inconsistencies including the editor language indicators, all the different missing/additional '-' characters, files that center the file name, missing trailing `===//` (mostly caused by clang-format breaking the line).

We understand that people may be surprised that we're moving the header entirely to discuss the new license. We checked this carefully with the Foundation's lawyer and we believe this is the correct approach.
Essentially, all code in the project is now made available by the LLVM project under our new license, so you will see that the license headers include that license only. Some of our contributors have contributed code under our old license, and accordingly, we have retained a copy of our old license notice in the top-level files in each project and repository.

Summary: This converts LLDB's logging to use llvm streams instead of lldb_private::Stream and friends. The changes are mostly straight-forward and amount to s/lldb_private::Stream/llvm::raw_ostream.
The part worth calling out is the rewrite of the StreamCallback class. Previously this class contained a per-thread buffer of data written. I assume this had something to do with it trying to make sure each log line is delivered as a single event, instead of multiple (possibly interleaved) events. However, this is no longer relevant as the Log class already writes things to a temporary buffer and then delivers the message as a single "write", so I have just removed the code in question.

Firstly, merging this particular commit into a downstream fork may be a huge effort. Alternatively, it may be worth merging all changes up to this commit, performing the same reformatting operation locally, and then discarding the merge for this particular commit. The commands used to accomplish this reformatting were as follows (with current working directory as the root of the repository):
find . \( -iname "*.c" -or -iname "*.cpp" -or -iname "*.h" -or -iname "*.mm" \) -exec clang-format -i {} + find . -iname "*.py" -exec autopep8 --in-place --aggressive --aggressive {} + ;
The version of clang-format used was 3.9.0, and autopep8 was 1.2.4.
Secondly, “blame” style tools will generally point to this commit instead of a meaningful prior commit. There are alternatives available that will attempt to look through this change and find the appropriate prior commit. YMMV.

All logging channels now make one and only one instance that is kept in a global pointer which is never freed. This guarantees that logging can correctly continue as the process tears itself down.

Found a race condition when sending async packets in the ProcessGDBRemote.
A little background: GDB remote clients can only send one packet at a time. You must send a packet and wait for a response. So when we continue, we obviously can't hold up the calling thread waiting for the process to stop again, so we have an async thread in the ProcessGDBRemote whose only job is to run packets that control the inferior process. When you send a continue packet, the only packet you can send is an interrupt packet (which consists of sending a CTRL+C (or a '\x03' byte)). This then stops the inferior and we can send the async packet, and then resume the target. There was a race condition that often happened during stepping where we are doing a source level single step which consists of many instruction steps and a few runs here and there when we step into a function. So the flow looks like:
inst single step inst single step inst single step inst single step inst single step step BP and run inst single step inst single step inst single step
Now if we got an async packet while the program is running we get something like:
send --> continue send --> interrupt recv <-- interrupt stop reply packet send --> async packet recv <-- async response send --> continue again and wait for actual stop
Problems arise when this was happening when single stepping a thread where we would get:
send --> step thread 123 send --> interrupt send --> stop reply for thread 123 (from the step)
Now we _might_ have an extra stop reply packet from the "interrupt" which we weren't checking for and we could end up with:
send --> async packet (like memory read!) recv <-- async response (which is the interrupt stop reply packet)
Now we have the read memroy reply sitting in our buffer and waiting to be used as the reply for the next packet...
To further complicate things, the single step should have exited the async thread since the run control is finished, but now it will continue if it was interrupted.
The fixes I checked in to two major things: - watch for the extra stop reply if we need to - make sure we exit from the async thread run loop when the previous run control (like the instruction level single step) is finished.
Needless to say this makes very fast stepping in Xcode much more reliable.

Switching back to using std::tr1::shared_ptr. We originally switched away due to RTTI worries since llvm and clang don't use RTTI, but I was able to switch back with no issues as far as I can tell.
Switching back to using std::tr1::shared_ptr. We originally switched away due to RTTI worries since llvm and clang don't use RTTI, but I was able to switch back with no issues as far as I can tell. Once the RTTI issue wasn't an issue, we were looking for a way to properly track weak pointers to objects to solve some of the threading issues we have been running into which naturally led us back to std::tr1::weak_ptr. We also wanted the ability to make a shared pointer from just a pointer, which is also easily solved using the std::tr1::enable_shared_from_this class.
The main reason for this move back is so we can start properly having weak references to objects. Currently a lldb_private::Thread class has a refrence to its parent lldb_private::Process. This doesn't work well when we now hand out a SBThread object that contains a shared pointer to a lldb_private::Thread as this SBThread can be held onto by external clients and if they end up using one of these objects we can easily crash.
So the next task is to start adopting std::tr1::weak_ptr where ever it makes sense which we can do with lldb_private::Debugger, lldb_private::Target, lldb_private::Process, lldb_private::Thread, lldb_private::StackFrame, and many more objects now that they are no longer using intrusive ref counted pointer objects (you can't do std::tr1::weak_ptr functionality with intrusive pointers).

Cleaned up code that wasn't using the Initialize and Terminate paradigm by changing it to use it. There was an extra parameter added to the static accessor global user settings controllers that wasn't needed. A bool was being used as a parameter to the accessor just so it could be used to clean up the global user settings controller which is now fixed by splitting up the initialization into the "static void Class::Initialize()", access into the "static UserSettingsControllerSP & Class::GetSettingsController()", and cleanup into "static void Class::Terminate()".
Also added initialize and terminate calls to the logging code to avoid issues when LLDB is shutting down. There were cases after the logging was switched over to use shared pointers where we could crash if the global destructor chain was being run and it causes the log to be destroyed and any any logging occurred.

Modified all logging calls to hand out shared pointers to make sure we don't crash if we disable logging when some code already has a copy of the logger. Prior to this fix, logs were handed out as pointers and if they were held onto while a log got disabled, then it could cause a crash. Now all logs are handed out as shared pointers so this problem shouldn't happen anymore. We are also using our new shared pointers that put the shared pointer count and the object into the same allocation for a tad better performance.

Updated the lldb_private::Flags class to have better method names and made all of the calls inlined in the header file for better performance.
Fixed the summary for C string types (array of chars (with any combo if modifiers), and pointers to chars) work in all cases.
Fixed an issue where a forward declaration to a clang type could cause itself to resolve itself more than once if, during the resolving of the type itself it caused something to try and resolve itself again. We now remove the clang type from the forward declaration map in the DWARF parser when we start to resolve it and avoid this additional call. This should stop any duplicate members from appearing and throwing all the alignment of structs, unions and classes.
