[libc++] Refactor the Windows and MinGW implementation of the locale base API (#115752)

This patch reimplements the locale base support for Windows flavors in a
way that is more modules-friendly and without defining non-internal
names.

Since this changes the name of some types and entry points in the built
library, this is effectively an ABI break on Windows (which is
acceptable after checking with the Windows/libc++ maintainers).

[libc++] Format the code base (#74334)

This patch runs clang-format on all of libcxx/include and libcxx/src, in
accordance with the RFC discussed at [1]. Follow-up patches will format
the benchmarks, the test suite and remaining parts of the code. I'm
splitting this one into its own patch so the diff is a bit easier to
review.

This patch was generated with:

find libcxx/include libcxx/src -type f \
| grep -v 'module.modulemap.in' \
| grep -v 'CMakeLists.txt' \
| grep -v 'README.txt' \
| grep -v 'libcxx.imp' \
| grep -v '__config_site.in' \
| xargs clang-format -i

A Git merge driver is available in libcxx/utils/clang-format-merge-driver.sh
to help resolve merge and rebase issues across these formatting changes.

[libcxx] [Windows] Use the standard vsnprintf instead of _vsnprintf

In ancient Microsoft C runtimes, there might only have been
a nonstandard `_vsnprintf` instead of the standard `vsnprintf`, but
in modern versions (the only ones relevant for libc++), both
are available.

In MinGW configurations built with `__USE_MINGW_ANSI_STDIO=1` (as it
is built in CI), `vsnprintf` provides a more standards compliant
behaviour than what Microsoft's CRT provides, while `_vsnprintf` retains
the Microsoft C runtime specific quirks.

Even if these comments have a benefit in .h files (for editors that
care about language but can't be configured to treat .h as C++ code),
they certainly have no benefit for files with the .cpp extension.

[SystemZ][z/OS] vasprintf fix libc++

The aim is to use the correct vasprintf implementation for z/OS libc++, where a copy of va_list ap is needed. In particular, it avoids the potential that the initial internal call to vsnprintf will modify ap and the subsequent call to vsnprintf will use that modified ap.

Update more file headers across all of the LLVM projects in the monorepo
to reflect the new license. These used slightly different spellings that
defeated my regular expressions.

We understand that people may be surprised that we're moving the header
entirely to discuss the new license. We checked this carefully with the
Foundation's lawyer and we believe this is the correct approach.

Essentially, all code in the project is now made available by the LLVM
project under our new license, so you will see that the license headers
include that license only. Some of our contributors have contributed
code under our old license, and accordingly, we have retained a copy of
our old license notice in the top-level files in each project and
repository.
