This patch replaces (llvm::|)Optional< with std::optional<. I'll post a separate patch to clean up the "using" declarations, #include "llvm/ADT/Optional.h", etc.

[clangd] Factor out the heuristic resolver code into its own class
The patch also does some cleanup on the interface of the entry points from TargetFinder into the heuristic resolution code.
Since the heuristic resolver is created in a place where the ASTContext is available, it can store the ASTContext and the NameFactory hack can be removed.

[clangd] Remove Tweak::Intent, use CodeAction kind directly. NFC
Intent was a nice idea but it ends up being a bit awkward/heavyweight without adding much.
In particular, it makes it hard to implement `CodeActionParams.only` properly (there's an inheritance hierarchy for kinds).

Make llvm::StringRef to std::string conversions explicit.
This is how it should've been and brings it more in line with std::string_view. There should be no functional change here.
This is mostly mechanical from a custom clang-tidy check, with a lot of manual fixups. It uncovers a lot of minor inefficiencies.
This doesn't actually modify StringRef yet, I'll do that in a follow-up.
