Remove $FreeBSD$: one-line sh pattern

Remove /^\s*#[#!]?\s*\$FreeBSD\$.*$\n/

Don't preprocess with tbl(1).

$Id$ -> $FreeBSD$

Revert $FreeBSD$ back to $Id$

This will make a number of things easier in the future, as well as (finally!)

Make all these format under the new scheme. I haven't had a chance to
test the -me stuff, so it's still disabled for now.

Make it make-able by adding a rule for paper.ps
