Add a vparamval field to Nameblock struct -- fix up klugy sharing between
Paramblock and Nameblock structs.

Added Jerry Berkman's changes to provide a definition for 'dblflag'
and to ifdef the Fortran 66 compatibility flags; also added an external
declaration for intrconv().
