4.4BSD snapshot (revision 8.1); add 1993 to copyright

get the const's right

bring up to date with POSIX 1003.2 D11.2; still broken, but at least
the return values are right, now

Add include files to get prototype declarations, and fix bugs found.

rearrange to make ANSI compilers happy;
f() { char *g(); } is an implicit extern declaration for g().
