changes to work under a V6 system -- mostly compilation flags

fix bug in file modes when mailing to files that didn't exist before;
add an 'F' line to the .cf file that will read class entries from
another file, given a scanf string to do the parsing.

cleanup, commenting, linting, etc.

plug assorted security holes

drop M_FINAL, change some other defined constant names

clean up some header information; don't reopen tempfile as
stdin (to fix some problems with saving mail on interrupts)

add header fields from new BB&N proposal ICST/CBOS - 80-2,
"Specification of a Draft Message Format Standard" for NBS.

but works better for the usual case; fix some quoting problems
